The Community Intervention Coordinator (CIC) is responsible for providing support and supervision services to youth on the trajectory for involvement in the youth criminal justice system. The overarching goal is to work with youth to reduce the potential for involvement in crime, while enhancing pro social engagement with school, community and family. Fundamental to this role is to assist youth in their physical, social, emotional, educational, recreational and vocational development that promotes their increased independence, allowing them to maintain connection with their natural family and function more successfully in their schools and community.

The Agency supports practice through a trauma informed lens, placing priority on the participant’s safety, choice and self-determination. The CIC is responsible for applying Agency Values and Code of Ethics to guide decision-making and professional behavior and complying with CARF accreditation standards, Agency policies, procedures and practices.

Wage: $25/hour
Status: Full Time
Hours: 40hrs/week
Shift: Monday to Friday, variable schedule based on program needs, between 8am and 6pm
Location: Various locations across South Vancouver Island

Key Duties and Responsibilities

• Acts as an advocate-liaison for youth and families in School District 62 by attending school meetings, supporting return-to-school plans, assisting parents in requesting additional support, and coordinating with school staff during transitions
• Maintains connection or re-connects youth and families to Middle and Secondary School and/or other viable education alternative
• Provides individualized, family-centered support by determining needs, developing social, life and coping skills, addressing food security through resources like food banks and backpack programs, and connecting youth and families to mental health, parenting, and community services
• Provides short-term crisis support by responding to situations such as school altercations, suicidal ideation, substance use or placement in care and de-escalating situations, offering safe transportation, conducting risk assessments, creating safety plans and connecting youth to appropriate resources
• Provide youth and families with long-term action planning by setting goals and collaboratively creating steps to achieve them through gradual support, skill-building and confidence development
• Manages referrals from multiple referral sources (school-based, community-based and self-referrals)
• Determine appropriate actions and service intensity based on referral information and consultation, creating a service plan to address needs; provides follow-up support as required for all youth and families
• Implements program activities that combine: individual and small group support, experiential life skills development, skills for independence, substance use education and supportive interventions, etc.
• Provides interventions using supportive, evidence-based practices (e.g. solution-focused techniques, motivational interviewing, trauma informed care), reviews and evaluates youth progress and modifies approaches and plans to support progress toward individual goals
• Develops and facilitates group topics/sessions to meet the needs of the program participant group based on ability, age, time of year and activities
• Maintains a caseload of up to 20 youth with varying degrees of intensity from take to follow-up
• Implements and co-facilitates groups of up to 12 participants on various psycho-social topics based on the needs of the youth
• Acts as an advocate, mentor and pro-social role model, providing guidance and brief, solution-focused support
• Perform other duties as assigned or requested, which may include tasks not listed in this position description but are within the scope of the position, program, and/or department
• Connects youth and families to specific resources and support people/services, based on expressed and assessed need
• Identifies resources and services that are congruent with the participants overall service plan and goals
• Works with the Wraparound team, applying the Wraparound framework to assist youth and families, create support teams, and develop strengths-based action plans.
• Receives referrals from School Personnel for CYIP youth
• Initiates contact with youth accepted into the service to begin assessment and planning for intake
• Maintains regular contact with School Counsellors, Social Workers and other community resources as required
• Actively participates in regular supervision and team meetings to receive feedback, reflect on work, discuss cases, debrief critical incidents, identify training needs and review policies, procedures, and case management strategies to ensure effective and consistent service delivery
• Attends regular team meetings as established
• Maintains all administrative duties and responsibilities to accreditation standard
• Participates in school-based meetings as an advocate for youth and families, collaborating with the school team to develop effective strategies for supporting the youth’s success
• Communicates effectively, timely and appropriately with school personnel
• Fosters positive working relationships by setting clear professional boundaries, maintaining respectful and supportive communication with youth, families and the case management team and collaborating with all parties to support the youth’s well-being
• Attends integrated case conferences to represent the youth, review progress, identify successes and challenges, gather feedback on strategies and collaborate with the team on transition planning.
• Supports youth participation in rehabilitation and intervention programs by maintaining ongoing contact during their treatment or detox program, ensuring continued engagement and strengthening their support network for after program completion
• Assists youth in complying with court-ordered reparative conditions by attending probation officer appointments, accompanying them to school and supporting attendance at other required appointments
• Identifies and supports recreational, social and/or cultural activities to facilitate youth disengagement from anti- social peers and activities
• Transports youth to appointments and activities
• Supports youth in making healthy lifestyle choices by providing education on nutrition, meal prepping, participating in physical activities, guiding them with healthy coping strategies and engaging in conversations about avoiding substance use and negative life choices; helps youth develop routines for hygiene, sleep, nutrition, and exercise
• Facilitates and promotes the development of advocacy skills by coaching youth on how to express their needs, identify their strengths and communicate effectively with authority figures; supports youth in building confidence to advocate for themselves in various settings, such as case conferences
• Identifies and implements a follow-up strategy of planned support for up to 90 days after discharge
• Acts as an advocate, mentor and pro-social role model, providing guidance and brief, solution-focused support
• Encourages youth to include other professionals and supports in the development and implementation of the service/transition plan, when appropriate
• Assesses situations accurately to mitigate risks
• Identifies strategies to deal with potentially dangerous situations; i.e., suicide threats, AWOL’s, aggressive behaviour
• Identifies safety risks and recommends the appropriate involvement/steps
• Ensures participant has resources to maintain their safety and completes safety plans with youth, as appropriate
• Complies with Critical Incident reporting as per Agency policies and procedures
• Oversees the Wraparound Process for each client by coordinating their support team, ensuring appropriate professionals are engaged and facilitating regular Integrated Case Management meetings to address needs, goals, and resources
• Maintains up-to-date documentation of case files;
• Identifies and implements effectiveness measures for the program in consultation with the school district’s Safe School Coordinator
• Completes necessary service delivery documents to BGCSVI standards
• Completes and stores records according to Agency confidentiality policies and applicable Provincial and Federal legislation
• Debriefs with youth regarding program participation and involvement in follow-up interviews
• Provides youth with an Exit Survey on or before their discharge date
• Meets with youth on a monthly basis to review identified service plan and goals
• Analyzes outcome data to identify trends on a quarterly basis

Minimum Qualifications

• Diploma in Social Work or Youth Justice
• 1-year experience working with vulnerable youth with complex needs
• Basic First Aid certification and CPR C
• Valid driver’s license and reliable vehicle
