Calgary Progressive Lifestyles Foundation (CPLF) is a family owned and operated registered not-for-profit charitable organization that serves people with disAbilities. Founded in 1989, today CPLF is considered one of Calgary's larger premier Service Providers supporting people with disAbilities at work, at home, and in the community.
As a Case Manager, you will be managing CPLF support teams to provide quality supports for our clients.
Hours of work are Monday to Friday 8:00am – 4:00pm. At CPLF we utilize a hybrid working model where we are in the office twice per week and 2 Fridays per month.
Key Responsibilities:
- Working with Complex Needs with more than 1 disability and/or multi-system involvement individuals.
- Reporting to funders and CPLF management which includes but not limited to composing various reports, including Risk Assessments, Abuse Reports, Critical Incident Reports, and Individual Service Plans, etc.
- Closely monitor quality service provision to ensure the individuals’ social, physical, cultural, and emotional needs are adequately met.
- Scheduling and completing monthly meetings with clients, support staff, guardians and / or external professionals.
- Maintaining client and staff safety and well-being. Coordinating with our Occupational Health and Safety Committee.
- Annual Job Performance Reviews of the staff you are supervising.
- Renewing and updating budgets for funding, including completing annual reviews to renew services and preparing proposals for new and / or updated services.
- Submitting urgent reports to funders in crisis situations, including allegations of abuse. Coordinating with our Abuse Committee.
- Developing and monitoring the use of behavioral tracking documentation, Support Protocols, and Support Plans.
- Coordinating with Human Resources about staffing and support with Disciplinary Actions, etc.
- Coordinating communication between stakeholders within support teams.
- Assisting individuals access available community-based resources for their various needs.
Mandatory Qualifications:
- Excellent time management, conflict resolution, critical thinking and crisis management skills.
- Excellent written and oral communication skills.
- Post Graduate Education.
- Must have a valid class 5 license and a vehicle.
- Must be able to provide a police clearance with a vulnerable person's search upon hiring.
- Must be computer literate and competent with the use of Microsoft Office tools.
Preferred Qualifications:
- Experience working with Persons with Developmental Disabilities (PDD), Harm Reduction, FASD, Complex Needs, ASIST, Addictions, Indigenous people, disability diagnosis including physical, cognitive, and behavioral diagnoses.
- Experience with social work and/or health care.
- Experience working with disabilities.

Pay: From $34.65 per hour
