Job Description: Reporting to the Manager of Rehabilitation and Transitional care, the Assessor Coordinator is responsible for the full scope of case management services for clients in Transitional or Convalescent Care. The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluation, and termination/discharge. This is done through collaboration with family, caregivers, and other members of the multi disciplinary team. The Assessor Coordinator liaises with other services in the Regina area, in an endeavor to provide the right service at the right time and in the right place. This position reports directly to the Manager for daily clinical supervision. This position collaborates with acute care, rehabilitation, home care, long term care, mental health and an array of government and non government agencies to assess clients' needs and coordinate services to meet those identified needs.

- Valid Class 5 driver's license

- Saskatchewan Association of Social Workers (SASW)

- Bachelor of Nursing is acceptable
