Qualifications
Completion of an addictions or social work diploma. A minimum of two years of experience working with clients in the field of mental health and/or addictions. If applicable, this position requires a minimum of 3 years continuous sobriety.
Duties & Responsibilities
A. **ADMINITRATION DUTIES
1. Accurate and secure file management
2. Accurate FYI support notes for all pertinent interactions with clients.
3. Enter Session notes, and complete forms and tabs as necessary.
4. Approve and enter passes and special permissions. Including relevant information from collaterals.
5. Completion of bi-weekly or monthly Progress Reports and submission to CFS Worker (obtaining information from all relevant team members).
6. Completion of Incident Reports according to the Incident Reporting Procedure. Immediately document and report all critical incidents to the Clinical or Executive Director.
B. **CLIENT RELATIONS
1. Positive interaction with residents to ensure their needs are met (physical, mental, emotional, and spiritual).
2. Meet residents where they are at and provide and open-minded and trusted ear for them to feel safe when they are ready to ask for help.
3. Create healthy boundaries and guide residents towards positive behaviors to get needs met.
4. Provision of resident binders and program material required for healthy living, awareness and day programming.
5. Support the scheduling of required appointments, probation meetings, medical and dental, school and parenting requirements, and social worker meetings. Ensure that transportation is available through the calendar and communication with the Transportation Worker.
6. Maintain and monitor all medical and dental appointments, recording outcomes, and ensure that all follow up requirements are met. Identify and arrange appropriate referrals to promote the wellness of the family (with the exception of child development referrals)
7. Identification of ongoing resident needs, including, legal, medical, psychiatric, financial, funding, identification, medication management, and health benefits.
8. Meeting with residents for one-on-one support and case planning. Support residents in the development of SMART goals. Ongoing monitoring and evaluation, identifying barriers and reframing new goals.
9. Ensure that residents have all required supports (income assistance, counsellors, sponsor, Dr. Appointments, etc.)
10. Identify 12-Step meeting needs and support attendance through in-home support.
11. Monitor scheduled reassessments and repeating SSP.
12. Attend Child and Family Services and wrap around meetings.
13. Monitor progress and programming expectations. (e.g. attendance, step work, adherence to the Conditions of Residency, engagement and independence)
14. Discuss, plan and support all transitions, including movement from the Family Healing Center to the Family Complex and transitions out of the program. Whenever possible, collaborate with appropriate staff, the resident’s safety network, community agencies and other collaterals.
15. Monitor and facilitate healthy spending through awareness and budgeting. Utilization of a weekly budget and the monthly review of bank statements will bring awareness. For those in Family Treatment, ensure that all budgeted monies are available for Client Support Workers to access.
16. For sober living residents, complete treatment applications when necessary.
C. **HOME ENVIRONMENT
1. Promote a positive attitude and healthy living environment for all residents. Through weekly meetings, identify concerns and work collaboratively to address.
2. Monitor home and room cleanliness. For Sober Living residents, ensure the completion of chores.
3. Coordinating and collaborating with the Client Supervision and Support Workers and the Family Mentor with regards to case plan, client or children needs and scheduling.
D. **HUMAN RESOURCES
1. Ensure consistent implementation and adherence to approved policies.
2. Support effective internal communication strategies.
3. Foster positive organizational culture through change management techniques.
4. Maintain a positive work environment.
E. **HEALTH AND SAFETY
1. Ensure the health and safety of all clients, staff, volunteers, and others visiting the center.
2. Monitor clients to ensure they are not under the influence of drugs or alcohol.
3. Conduct random client urinalysis drug and alcohol screens, when necessary or scheduled.
4. Schedule any required house repairs and completion of maintenance requests.
E. OTHER DUTIES AS REQUIRED
Job Types: Full-time, Permanent
Pay: $21.00-$23.00 per hour
