The Wellness Program Coordinator leads and coordinates wellness services for seniors age 65+. This role ensures seniors get the support they need - from personal care to social engagement - while building community connections and improving overall well-being.
Duties
- In-Home Support - Coordinate personal care and home help; manage medication reminders and follow up .
- Transitional Care - Support hospital-to-home transitions; connect clients to respite and healthcare services.
- Community Engagement - Conduct wellness check-ins and home visits; organize social, health, home safety, and recreational workshops and events; organize peer mentorship programs / mutual support groups; coordinate transportation (Wheel-Trans, volunteer drivers).
- Technology Support - Access to tablets with telehealth capabilities; train seniors to access digital services and tools.
- Financial & Housing Support - Assist with pensions, benefits, and housing applications; offer financial literacy support.
- Support for Isolated Seniors - Provide advance care and life planning, grief support, and peer connections.
Requirements
- University degree in social work, nursing or gerontology.
- Experience in program coordination, social services, or healthcare (seniors-focused preferred).
- Knowledge of senior wellness needs and local services.
- Strong communication and interpersonal skills.
- Able to work both independently and in a team.
- Bilingual (English and Chinese) and access to a vehicle is an asset.

Pay: $45,500.00-$58,240.00 per year
