Position Overview
Responsible and accountable to the Manager of Interprofessional practice for the provision of patient care, coordination of service delivery, education, professional/program development and quality assurance duties.
1. Provides comprehensive assessment and counselling of patients in the renal program, both in dialysis and in renal clinics
- Assesses psychosocial needs of patients and families
- Advocates for services to meet specific patient's needs
- Develops treatment plans for patients/families
- Assesses and counsels patients in the area of new diagnosis and chronic illness management
- Home visits may be required
Education
- Graduate of an accredited university program in Social Work
- Minimum Masters of Social Work required
- Registered with the Ontario College of Social Workers & Social Service Workers or a candidate for registration
Experience
- Recent experience with renal patients in a dialysis and/or renal clinic setting required
- Excellent working knowledge of resources available to patients with chronic illness
- Skilled in Chronic Illness management counselling and education
- Excellent interpersonal and communication skills
- Willingness to participate/lead health service councils and task groups
- Motivated to practice in accordance with the mission and vision of hospital/health service
- Good work and attendance record required
