About Us
Since 2011, the Humber River Family Health Team (HRFHT) has partnered with over 43 physicians across six sites to deliver high-quality, comprehensive care to our community. Our interdisciplinary team comprising nurse practitioners, registered nurses, social workers, dietitians, chiropodists, and a pharmacist, works collaboratively with community partners to provide inclusive, culturally responsive health services. We are dedicated to health promotion, disease prevention, and addressing the social determinants that shape health outcomes.
Position Overview
The Social Worker provides short-term psychotherapy, systems navigation, and group support to patients and families. Working within a primary care setting, the Social Worker plays a key role in program development, care transitions, and coordination with external service providers. This role reports to the Executive Director and follows the standards and ethics outlined by the Ontario College of Social Workers and Social Service Workers.
Key Responsibilities
- Deliver short-term (max 12 sessions) therapy for patients with mild to moderate anxiety, depression, and trauma.
- Conduct intake assessments and develop individualized treatment plans.
- Provide therapeutic interventions: supportive counselling, psychoeducation, crisis management, and evidence-based modalities.
- Monitor progress and document outcomes using EMR.
- Manage discharge plans, referrals, and post-discharge follow-ups.
- Facilitate therapeutic and educational groups.
- Collaborate with interdisciplinary teams and attend regular team meetings.
- Engage in program evaluation and development initiatives.
Qualifications
- Master’s degree in Social Work from an accredited institution.
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
- Clinical experience in CBT, with additional training in DBT, EFT, MBSR, and Motivational Interviewing considered assets.
- Knowledge of primary care systems and physician-led care models.
- Demonstrated commitment to patient-centred, trauma-informed, and non-judgmental care.
- Strong understanding of anti-racism/anti-oppression principles and health equity.
- Familiarity with social determinants of health and community-based resources.
- Experience supporting individuals facing gender-based violence, psychosis, substance use, and end-of-life care.
- Up-to-date with best practices and committed to ongoing professional development.

Organizational Duties
- Maintain client records in EMR and contribute to data collection/reporting.
- Participate in team building, quality improvement, and case review meetings.
- Uphold HRFHT policies, procedures, and professional standards.
Confidentiality
- Strict adherence to privacy legislation (Freedom of Information and Protection of Privacy Act).
- Responsible for protecting client and organizational confidentiality at all times.
Additional Details
- Position requires travel between sites and occasional meetings off-site.
- The successful candidate will be employed by the Humber River Family Health Team and will have no employment relationship with Humber River Hospital.
