Job Description

Accountable to the Director, Integrated Health Systems, the Nurse Practitioner led Outreach Team (NLOT) supports residents of long term care (LTC) facilities in the region, helping to reduce unnecessary emergency department visits and inpatient admissions.  The Nurse Practitioner (NP)  will demonstrate a high level of autonomy and expert skill in formulating clinical decisions when supporting residents within the Central West LTC, and potentially other congregate living settings in the community as part of the integrated health portfolio. The Nurse Practitioner will work to full scope of practice, utilizing all nurse practitioner core competencies (client care, quality improvement and research, leadership and education) alongside their NLOT counterparts. .

Hours: Currently days including the potential for weekends, evenings and statutory holidays based on program needs (subject to change in accordance with operational requirements)

Note: This position is temporary contract for 12 months.

Minimum: $62.16 per hour
Maximum: $77.42 per hour
