The Community Outreach Counsellor (Long Term Care) assists clients to identify, understand and overcome personal problems using a short-term counselling model.
The incumbent provides social work support to seniors in long-term care and their family members at Fairmount Homes. The Community Outreach Counsellor (LTC) will provide compassionate care to residents facing multiple challenges and barriers (including depression, dementia-related behaviours, disability, loss, isolation, palliative care and loneliness). The Community Outreach Counsellor (LTC) will also support family members, friends and service providers in understanding the impact of these challenges on their loved ones and providing support and referrals to resources.
Overall performance will be assessed through client service delivery and satisfaction. The Community Outreach Counsellor’s (LTC) duties and responsibilities will promote the highest standards in these areas.
1. Ensure client needs are met by:
· Assessing the needs of residents and family members who are referred by the care staff at Fairmount and establish personal goals and service plan.
· Using a brief-service, solution-focused and supportive model to determine follow-up support and resources for clients if required. This may include referrals to further counselling for residents, provision of resources or recommendations for in-house activities. or referrals to other community based support programs
· Referring residents and / or their families to other community resources as appropriate and providing support to assist them in accessing these resources (e.g. Alzheimer’s Society, Bereaved Families of Ontario etc.)
· Reviewing and discussing cases in supervision sessions with the Clinical Director
2. Ensure Fairmount Homes is being supported by:
· Ensuring clients complete an evaluation tool pre and post-service delivery and the client satisfaction survey.
· Completing all statistical reporting accurately and within deadlines.
· Quarterly meetings between Resolve and Fairmount Home leadership to ensure that targeted goals for long-term care are being met.
3. Maintain and encourage professional standards by:
· Participating in continuing education opportunities as appropriate.
· Participating in Clinical Team Meetings.
· Ensuring that membership in professional association(s) is current (for this position, the outreach social worker must be a registered social worker with the Ontario College of Social Workers and Social Service Workers).
· Keeping abreast of current policies and regulations regarding the long-term social work program.
4. Maintain links with other resources in the community ensuring personal knowledge of services provided is current.

1. EDUCATION & EXPERIENCE
· Post-secondary education in social work is required by this position (BSW or MSW)
· Registration with the Ontario College of Social Workers & Social Service Workers (mandatory)
· Three to five years clinical experience would be an asset,
· Specific training and/or clinical experience in geriatric social work is preferred,
· On-going participation in continuing education opportunities.
2. KNOWLEDGE, SKILLS AND ABILITIES
Incumbent must have working knowledge of computers and be proficient in using MS Office suite and the internet. Experience with client database and scheduling software preferred. The ability to quickly learn and become proficient with new types of software an asset as Fairmount Homes uses a specific medical database.
Incumbent must have good time management, assessment, prioritization, and problem-solving skills.
Must have excellent verbal and written skills and the ability to establish rapport and trust with residents under sometimes difficult circumstances. The social worker must have a compassionate, gentle and patient nature to allow for residents to feel heard and supported.
Commitment to task, and the ability to maintain confidentiality are essential. Must have the ability to deal with change, to self-nurture, and to work independently and in a team to achieve results.
Incumbent must have good judgement and know when to seek support from peers or supervisors.
The ability to research and incorporate resource material and to research and establish contacts with community resources is essential.
3. LEVEL OF DIRECTION RECEIVED
Incumbent performs work as defined by established internal written methodology.
Incumbent must comply with all pertinent legislation and associated regulations. This may include but is not limited to: PIPEDA, Occupational Health & Safety Act, Mental Health Act, Children and Family Services Act, and code of ethics for professional associations.
Incumbent consults with the Director of Clinical Programming or the Clinical Team Lead regarding case management or supervisory issues such as clients at risk of suicide/homicide or possible “at harm” situations, unfamiliar situations, borderline ethical situations or safety issues.
4. WORKING CONDITIONS Physical conditions:
Risk associated with face-to-face meeting with clients, prolonged sitting and use of keyboard and mouse, some travel may be necessary.
Psychological conditions:
Intense mental focus, secondary stress/trauma associated with meeting with clients under sometimes-severe emotional conditions.
5. OTHER REQUIREMENTS
· Driver’s license, access to a vehicle and proof of insurance.
· This position is based at 2069 Battersea Rd, Glenburnie, ON.
· Proof of appropriate vaccination for COVID-19 (is this required for Fairmount?)
· CPIC (Vulnerable Sector Check)
· Ability to speak French an asset* (reference our stipend for French speaking staff)
