We are seeking a dynamic Certified Diabetes Educator (CDE) to provide care management of C-health Diabetes and Endocrinology Clinic patients and provision of diabetes education group classes. The Certified Diabetes Educator (CDE) is responsible for assessment, development, implementation, and evaluation of client/patient care plans in chronic disease management. The CDE provides expertise to internal and external professionals and translates scientific literature to clients/patients, professionals, and the public appropriately. The CDE uses counselling, behavioural change theories/strategies, as appropriate to support patients in working towards their desired health outcomes.
Responsibilities:
Patient Education Responsibilities
- Provide individual and group education sessions to patients with diabetes.
- Assess patients’ knowledge and understanding of diabetes and adapt education plans to meet their specific needs.
- Teach administration of insulin effectively, including the titration and management as per medical directives.
- Teach self-monitoring of blood glucose (SMBG) effectively as per medical directives.
- Collaborate with the healthcare team and the patient to develop and implement comprehensive diabetes management plans.
- Assist in creating individualized plans to incorporate nutrition, activity, and medication as applicable for patient care plans.
- Employ information technology appropriately for patient care (Blood Glucose Meter, Continuous Glucose Meter, Insulin Pump Therapy).
- Monitor and adjust treatment plans as necessary to support patients in achieving optimal blood glucose control.
- Utilize behaviour change counselling techniques to support patients in working towards their health goals.
- Offer support to patients dealing with the challenges associated with diabetes and connect with relevant supports as indicated.
- Monitor patients’ blood glucose records and other relevant data in regular follow-up visits as indicated.
- Provide ongoing assessment of treatment effectiveness and review with physician as indicated.
- Assess patient readiness for insulin pump therapy.
- Manage patients on insulin pump therapy, as per medical directives.
- Stay up to date with the latest research and developments in diabetes management and treatments.
- Maintain your certified diabetes educator (CDE) qualifications.
- Plans and participates in relevant educational programs and workshops to increase professional competence and to meet personal needs and goals.
- Ensure complete and clear chart notes are completed within the clinic EMR (Accuro).
- Contribute to the development of patient handouts, as required.
- Provide consultation, as requested, to community groups, partners, and team members.
- Mentor nutrition volunteers, dietetic interns, and community CDEs, as required.
- Present at professional seminars, conferences and lectures as the opportunity arises.
- Assist in maintaining clinic medication samples along with other non-medicinal supplies.
- Assist in fostering strong external relationships with key contacts.
- Other duties as may be assigned by supervisor.
Qualifications, Experience & Requirements:
- Bachelor of Human Ecology, Foods, and Nutrition Major (or equivalent) and completed an accredited Dietetic Internship or Bachelor of Nursing
- Holds a valid RD license and is in good standing with the College of Dietitians of Alberta or holds a valid RN license and is in good standing with CARNA
- Exceptional written and verbal communication skills, with the ability to engage, inspire, and build trust
